A Clinical Reriew of Perforated Peptic Ulcers of Stomach and Duodenum

Abstract
This clinical review was made of 96 cases in which perforated gastric and duodenal peptic ulcers were treated surgically at the Pusan University Hospital from November 1956 to August 1956.
1) Perforation occurrrd in 60 cases (42.5%) out of 142 with gastirc ulcer and Perforation occurrrd in 93 cases(46.15 %) out of 18 with duodenal ulcer. The incidence of perforation was higher among those with gastric ulcer 60(42.25%) than in those with duodenal ulcer 36(46.15 %).
2) Peforation was commonest in persons in the 4th and 5th decades(43.75%). The sex distribution assumed a ratio of 47 to 1 with 94 males and 2 females. The lowest numher of cases occurred during March(4 cases) and August(4 cases) and the greatest number during September(919 cases) and January(16 cases). The duration of gastrointestinal Symptoms characteristic of peptic ulcer was 0-5 years in the majority(56.25%)
3) Subdiaphragmatic free was on plain abdomen x-ray study was noted in 65 cases(83.3%) out of 96 patients. Abnormal liver function was noted in 41 cases(53.9%). Of the 96 cases, 32(33.3%) the largest number belonged to blood group A.
4) The ratio of perforation in gastric and duodenal ulcers was 5 : 3. The pylorus region was perferated in 38 cases(39.5%) and the perforaticn was found in the first part of the doudenum in 35 cases (36.4%). Of peritoneal fluid cultures, 43(61.4%) were positive and streptcoccus viridans was the most common organism. In operative procedures, 35 cases had simple closures and 61 cases had immediate distal partial gastrectomies.
5) There were 12(12.5%) nearly postoperative complications while, still in hospital the results frere as follows 43(44.8%) of the patients made excellent progress, 27(28.1%) made good progress, 8(8.3%) made fair progrees and in 12(12.5%) the results were poor.
